Start with the following equipment:

and the following software:

-
System Tools
-
Utilities 1
-
Utilities 2
-
Printing Tools
Iomega Driver 4.2 (800K, write protect this)
Switches on Iomega ZIP drive

Copy all files from Hard drive to a ZIP disk as a backup.
Boot up with the System Tools disk.
Select hard drive and under Special Menu choose Erase Disk.
Make sure the right disk is going to be erased and click on Initialize.
Open System Tools disk and double click on the Installer.
Click Ok on the welcome screen.
Click Install on the Easy Install screen and from here you just keep feeding the Mac the disks it asks for. (In my case each of the OS 6.0.8 disks and the startup disk.)
When the installer tells you it was successful, click on Quit.
Under Special choose Shut Down
After the System Tools disk is ejected, click on Restart.
When the Mac is restarted, insert the ZIP driver diskette.
Open the hard drive folder and the Zip driver diskette.
Drag the ZIP driver from the diskette to the System Folder that is on the hard drive folder.
Under Special choose Shut Down.
After the ZIP driver diskette is ejected, click on Restart.
At this point you have a clean install and can add programs and files as needed.
